What is LASIK?

Laser In-Situ Keratomileusis (LASIK) is a surgery in which a laser is used to reshape the cornea to correct or reduce moderate to high levels of nearsightedness (Myopia), astigmatism and farsightedness (Hyperopia).  During the LASIK procedure a microkeratome is used to gently create a flap in the cornea. The flap is lifted and an extremely precise excimer laser is then used to shape the remaining tissue. The flap is then carefully set back into place, forming an occlusive seal by osmotic pressure, without stitches!

LASIK has emerged as one of the safest, most effective, and most popular vision correction procedures available.  Patients report an almost immediate improvement in their vision following the LASIK procedure. Most patients say they experience little or no discomfort during or after the procedure.

Zyoptix Custom Wavefront LASIK is the most advanced laser technology available.  Zyoptix Custom Wavefront LASIK  laser treatments are based upon the unique visual characteristics of your eye. Up until now, with glasses, contacts and conventional LASIK surgery, corrections were quite similar for each type of prescription.  Zyoptix Custom Wavefront LASIK involves measuring the eye from front to back with a special Zywave aberrometer, using what's called "wavefront" technology, to create a three-dimensional (3-D) scan of your eye. The information contained in the wavefront 3D map combined with our B&L Orbscan corneal topography creates a unique visual “fingerprint” for your eye which guides the laser in customizing the treatment to your personal visual system.

How much you see depends on what lower-order and higher-order aberrations you have; lower-order aberrations are also called refractive errors (i.e. nearsightedness, farsightedness and astigmatism).  All eyes have some degree of imperfections or aberrations. One way to measure the focusing errors of an eye is to measure its wavefront.  The Bausch & Lomb Zywave® II aberrometer provides correction of higher order aberrations through the 5th order, not just sphere and cylinder and has the largest FDA-approved optical zone.
No other laser platform in the US corrects higher order aberrations out to the 5th order.

Conventional LASIK treats these lower-order aberrations. How well you see can also depends on what higher-order aberrations you have; higher-order aberrations are irregularities other than refractive errors, and can cause such problems as decreased contrast sensitivity or night vision, glare, shadows and halos. Higher-order aberrations do not always affect vision.

Am I a Good Candidate for LASIK Surgery?
 

The ideal candidate for LASIK surgery is someone who wants to significantly reduce or even eliminate his or her dependence on glasses or contact lenses. In addition, ideal LASIK candidates should:
 

Be at least 18 years old
Have had stable vision for at least one year prior to surgery
Have a history of good ocular health
H
ave corneas of appropriate thickness to create a corneal flap
Have healthy lenses without signs of cataracts
LASIK is more dependent on surgical skill than
PRK. Take this into account when selecting a surgeon. Patients who have previously had Radial Keratotomy (RK) are often candidates for LASIK surgery.

 

Certain medical conditions such as diabetes, keratoconus, and lupus may affect candidacy. You should discuss your health as well as pregnancy and any medications you are taking with Dr. McMann during your LASIK evaluation.

What Are My Options If I Am Not Considered a Good Candidate for LASIK?

PRK (Photorefractive Keratoplasty)

PRK is a laser refractive surgery that provides an excellent alternative for patients who are not good candidates for LASIK. The primary difference between PRK and LASIK is that PRK involves the complete removal of the outer layer of corneal cells in order to access the tissue underneath. Once these cells have been removed, Dr. McMann uses a laser to reshape the cornea as he would with custom LASIK procedures. Although the outer layer of corneal cells will regenerate following the surgery, this results in a substantially longer healing period than with LASIK. Nevertheless, patients will experience generally excellent results once they have achieved their best vision.

The next step involves a series of very thorough diagnostic tests to determine whether your eyes are suitable for the treatment. There are two types of treatment, conventional LASIK and custom Zyoptix LASIK treatment.  The custom Zyoptix procedure is exactly the same as with conventional LASIK, except that the diagnostic tests are more complex, and with custom Zyoptix, your own unique special treatment program is created specifically for your eyes.

When you have decided to proceed, our highly-trained clinic staff will McMann Eye Institute - We have the necessary equipment to make all of the proper diagnostic test for your conditionperform many tests to ensure that you are suitable for Zyoptix. Once the area around your eyes has McMann Eye Institute - Eye Care Specialistbeen thoroughly cleaned, you will receive anesthetic eye drops that will completely numb your eyes to make sure that you feel no pain.  You will lie face up on a laser bed and your eyes and eyelids will be kept open, to make sure they don't get in the way of the laser beam. Dr. McMann will perform the surgery and the treatment should be over in 15-20 minutes.

McMann Eye Institute - LASIK ProcedureFirst, Dr. McMann will place a small device between your eyelids to ensure that you do not blink during surgery. Then an extremely precise instrument called a microkeratome is used to create a flap in the outer surface of the cornea.  To promote the safety and comfort of his patients, Dr. McMann uses the XP Zyoptix Microkeratome, McMann Eye Institute - Lasik - Creating the Flapthe most advanced and trusted microkeratome available. The flap is then carefully folded back out of the way. This procedure is generally painless although you may feel a very mild pressure from the vacuum pump.  To calm you, we have a clinician holding your hand and talking to you throughout the entire procedure.

The Laser Correction

Once the flap has been created, Dr. McMann will be able to access the misshapen cornea tissue underneath.  Flaws in the shape of the cornea are responsible for most visual errors; using the extraordinarily precise Bausch & Lomb - Technolas excimer laser, Dr. McMann will reshape your cornea according to the data collected during your pre-LASIK examinations.  Because the laser is programmed to treat corneal flaws based on an extremely detailed analysis of your specific eye, your Zyoptix Custom Wavefront LASIK procedure will be precisely tailored to your visual “fingerprint”, which significantly increasing your chances of improved vision.
         
McMann Eye Institute - LASIK Eye Procedure
 
The laser is programmed with your unique treatment instructions. Dr. McMann aligns your eye under the laser and asks you to stare at a red light. The laser's Eyetracker then visually locks on to your pupil. The laser treatment begins and usually lasts for 30 to 90 seconds. During the procedure the Eyetracker ensures that even if your eyes move the tiniest amount the laser will compensate for this movement and ensure that the treatment of your eye is completely accurate.

Replacing the Flap

McMann Eye Institute - LASIK Surgery - Your Care is OUR Top PriorityThe actual LASIK procedure requires only a minute or two per eye, after which the corneal flaps will be returned to place and left to heal without the need for stitches.  Dr. McMann will perform one more final check on your eyes under a second microscope in the surgery suite.

  1. What Happens after the Treatment?

After the procedure, your vision will be tested and you will be given medication to prevent infection and/or dry eyes. You will receive eye shields to prevent you from rubbing your eyes while asleep and sunglasses to protect your eyes from strain because they will be sensitive to bright light for a short time. Your eyes will not yet be clear enough for you to drive or even to use public transport, so have someone with you who can take you home.

Although most patients will be able to see well enough to drive following the procedure, it is best to arrange for someone to drive you to and from your appointment, as well as to and from your follow-up appointment with Dr. McMann the next day following your surgery.

During this follow-up, Dr. McMann will examine your eyes to assess the results of your LASIK procedure. You may feel some slight post-operative discomfort in the 24 hours following surgery, and your vision may require several months to fully stabilize; however, you should notice an improvement in your vision almost immediately.

The next day, when you go for your first post-operative check-up, you should have very good vision. But no matter how well you feel or how pleased you are with your eyesight immediately after treatment, it is McMann Eye Institute - Professional Careextremely important to follow your surgeon's instructions with regard to medication and general eye-care. You must attend all post-operative visits at day 1, 1 week, 1 month and 3 months after treatment , and we also recommend you see Dr. McMann at 6 and 12 months after surgery as well.

QuestionHow do I know if I am a good candidate for LASIK?

AnswerMost people who are nearsighted, farsighted or have astigmatism can benefit from laser or surgical vision correction. Based on a thorough evaluation of your eyes, Dr. McMann will make a specific recommendation about LASIK. His decision will be based on many factors including your age, vision stability, overall eye health, current prescription, and your expectations. QuestionDo I have to stop wearing my contact lenses prior to surgery?

AnswerYes, you should discontinue wearing soft contact lenses for 2 weeks and 4 weeks for hard or gas permeable lenses prior to your pre-op evaluation. Contact lenses warp or mold the shape of your cornea thus we need to allow for the cornea to return to its natural shape before measurements are taken.

QuestionHow long does the LASIK procedure take?

AnswerThe actual procedure takes between five and seven minutes per eye to perform, although the laser is on for just a matter of seconds. The average total time at the surgery center is about 1 - 1 1/2 hours on the day of your procedure.

QuestionDoes LASIK hurt?

AnswerMost patients report that the surgery is painless, or that they feel only a light pressure sensation. Before your procedure, you will be given anesthetic drops to completely numb your eyes. After your procedure, your eyes may feel gritty for a few hours. Some patients need a prescription pain reliever, but most people need only Tylenol or Advil and a little rest time.

QuestionWhat if I blink or move?

AnswerThis is a common concern with patients. There is a target light for you to focus on during the procedure. Dr. McMann will also monitor your eye position through a microscope to assure proper alignment. To make sure the laser treatment is properly centered, the laser that he uses has a sophisticated active eye tracker that follows your eye movements. It will only deliver laser treatment if your eye is in the correct position.

QuestionHow soon will I notice an improvement in my vision?

AnswerMost patients experience improvement in vision within 24 hours after their procedure, allowing them to resume most normal activities. You'll most likely be able to return to work within a day or two however, it may take several weeks for your vision to completely stabilize.

QuestionWhen will I be able to drive?

AnswerOn the day of your surgery it is important to have a driver take you home. Most patients drive the following day, or when their vision allows.

QuestionWhat should I expect after surgery?

AnswerYou may initially experience increased dryness, glare, and halos at night as well as some fluctuation in your vision. These symptoms are a normal part of the healing process, and usually resolve over time. You will use medicine drops as well as artificial teardrops during the post-operative period.

QuestionWhat are the possible complications?

AnswerAs with any surgical procedure, there are risks to LASIK. Potential complications include under or over-correction, corneal flap healing problems, dry eye, hazy vision, sensitivity to light, glare at night, loss of best corrected visual acuity, and infection. The majority of patients do not have complications. LASIK has a 99% success rate. This is a better outcome than almost any surgical procedure for any problem.   The majority of patients do not have complications.
 
By following instructions, your chances of having a problem are minimal. Infection is possible, but in order to avoid this, we encourage you to wash your hands with soap and water before getting near the eye or putting in the drops. The "flap" could rotate. This is why we have you wear protective goggles for 24 hours after surgery and at night for 2 weeks. Should the flap rotate, it can be re-positioned.
 
You can reduce your risk of complications by going to an experienced surgeon who has invested in advanced technology to determine whether you are a good candidate for LASIK. A sterile, humidity-controlled laser center will help ensure that the laser works properly.

QuestionShould I expect night vision problems?

AnswerIt's common to experience glare and halos at night for the first few months after your procedure. This typically diminishes over time. Ongoing problems can be caused by the use of older generation lasers and poor treatment designs. The new flying spot lasers, such as our Bausch and Lomb Technolas laser apply a much smoother treatment and blend the region between treated and untreated cornea better. The B&L laser provides the largest diameter treatment zone available, minimizing the risk of nighttime glare and halos. Very few of our patients report any night vision problems after their first month post-op.

QuestionHow long will the effect of LASIK last?

AnswerIn most cases, the treatment effect of laser vision correction is permanent, especially if your eyeglass prescription was stable before treatment. However, your eyes may change in the future due to other, unrelated conditions such as cataracts.

QuestionWhat are my restrictions after LASIK?

AnswerMcMann Eye Institute - Lasik and the good lifeThe only restrictions are no contact sports (tennis, racquet ball, baseball etc), surfing, swimming, hot tubs or eye make-up for 2 weeks after LASIK.  Golf, walking, running or bike riding (with protective glasses) is acceptable the day after the procedure. You should not rub your eyes for 6 weeks after LASIK surgery.

QuestionCan I have LASIK if I have diabetes or glaucoma?

AnswerYes, however these health conditions should be under control.

QuestionWhat are your fees?

AnswerAll of the LASIK procedures done by Dr. McMann are performed with the Bausch & Lomb Technolas® Zyoptix 217z excimer laser. The only pricing difference is whether you choose conventional laser ablation or Zyoptix Custom Wavefront ablation.

Our fee includes all pre-operative testing, the LASIK procedure, and 1 year of follow-up examinations done by Dr. McMann; we do not pass you off to another eye care professional.  Our fee is not based on your prescription and is only based on the level of technology you prefer.  Once you factor in all of the extra fees of other refractive surgeons, you will find that Dr. McMann’s rates are very competitive.
